Sutures:
6-0 vicryl for traction suture
7-0 prolene to anchor plate to sclera
9-0 nylon to anchor tube to sclera
7-0 vicryl to tie of tube, suture graft to sclera, close conjunctiva and tenons

Disposables:
22-gauge needle to enter anterior chamber for tube placement
15" super-sharp blade for paracentesis
Viscoelastic to maintain anterior chamber

Surgical instruments:
Heavy jaw needle holder x 2 (place traction suture & tie 7-0 vicryl sutures)
Medium jaw needle holder (place and tie 9-0 nylon to anchor tube)
.12 forceps (manipulate sclera)
.3 forceps (manipulate sclera, place plate into position)
blunt-tipped Westcott scissor (open conjuctiva and tenons layer)
Jameson muscle hook (to isolate rectus muscles)
Long Vannas scissors (cut sutures, cut tube with bevel)
Medium tying forceps (place tube into anterior chamber)
